Reporting Schedule
The following are the required reports and due dates for FY2013. Specific instructions and templates for each report will be made available on this page throughout the year. Please contact the state office if you have any trouble accessing or downloading any of the reporting information.
- Desk Audit- The last Thursday of the month for the following months; September 2012, December 2012, March 2013, and June 2013.
- Annual Report – October 31, 2012
- Corrective Action Plans (CAP) and Updates- Due Dates will be determined at the time the CAP is issued. Programs will be notified in writing.
- RFP or Continuation Application for FY 2014- Will be issued spring 2013. Due date is TBA, generally May-June.
